Course Enrollment Form

New office location for Registration Services: First Floor of the Jim D. Morris Center for Continuing Education at 301 South Jefferson, Springfield, Missouri. The building is accessible by the shuttle.

The Course Enrollment Form can be printed out and either mailed or faxed to the Extended Campus Registration office. To print the form you must have the Adobe Acrobat Reader on your computer.

Use the PRINT button on your browser or choose PRINT from the FILE menu to print the form on your printer. Fill it out and follow the registration instructions on the second page of the form.

Submitting Payment with Your Registration

The Course Enrollment Form allows you to submit your registration request and payment via credit card if you wish. Credit card payments may also be submitted online at (note the 24 hours advance required), or you may call our Bursar’s Office directly to submit payment at 1-417-836-5128.

Deadlines for submitting initial payment vary; the closer the opening date of the semester, the more immediate the payment requirement to avoid cancellation of your entire semester schedule of classes. As soon as the semester has opened, the initial payment is due along with your registration.

If you are registering through the Extended Campus’ Registration Services, we must process your initial registration before your payment will be accepted by the Bursar. To check if your initial registration has been processed, use your PIN and go to My Information and click the "Class Schedule" link or call us at 1-877-678-2005(Monday through Thursday 8am to 6pm or Friday 8am to 5pm).

Troubleshooting

If the form does not print correctly, try decreasing or increasing your font size. The default font size for Netscape and a font setting of "small" for Internet Explorer should be the appropriate font size for printing. For Netscape 6 users, click the PRINT icon in Acrobat Reader to print form if the above methods do not work.
